چکیده

To the Editor: In two recent publications in Anesthesiology, Dr. Eghbali’s group reports the attenuation of myocardial reperfusion injury in rodents by intralipid administered on reperfusion.1,2 Taken together with another study by the same group in which intralipid prevents and even rescues pulmonary hypertension,3 and the serendipitous landmark discoveries of lipid rescue therapy against bupivacaineinduced cardiotoxicity first in dogs4 and then humans,5 intralipid appears to have become a new magic bullet for cardioprotection. Nevertheless, many questions remain. Li et al.2 state that intralipid acts through the phosphorylation of Akt/extracellular signal-regulated kinase-1/glycogen In Reply: We thank Drs. Johnson and Rice for correcting our inadvertent error from our article1 and agree with their comments.

The Mechanism of Intralipid®-Mediated Cardioprotection Complex IV Inhibition by the Active Metabolite, Palmitoylcarnitine, Generates Reactive Oxygen Species and Activates Reperfusion Injury Salvage Kinases

BACKGROUND Intralipid® administration at reperfusion elicits protection against myocardial ischemia-reperfusion injury. However, the underlying mechanisms are not fully understood. METHODS Sprague-Dawley rat hearts were exposed to 15 min of ischemia and 30 min of reperfusion in the absence or presence of Intralipid® 1% administered at the onset of reperfusion. In separate experiments, the rea...

Perioperative cardioprotection

Although anaesthesia itself is now very safe, perioperative cardiac complications during non-cardiovascular surgery are a major cause of morbidity and mortality, because of the increasingly high underlying prevalence of cardiovascular disease. Fortunately, although there is no "magic bullet", pharmacological intervention can reduce the risk. In particular, current evidence strongly supports the...

Ehrlich's Magic Bullet - and the Curse of Injections

Paul Ehrlich, the inventor of the Magic Bullet died, regretting the slaughter of the First World War. As biology students in the early 1940’s we all knew one important number, 606, the name of the first man-made drug, Salvarsan or the Magic Bullet. A German doctor, Paul Ehrlich, had noticed that different dyes would stain tissues and even bacteria and parasites. He noticed that methylene blue s...

Comparison of the Effects of Intralipid 10% and 20% on Peripheral Catheter Ablation in a Pediatric Intensive Care Unit

Background: Energy deficit is a common and serious problem in pediatric intensive care units. Parenteral nutrition, either alone or in combination with enteral nutrition, can improve nutrient delivery in critically ill patients by preventing or correcting the energy deficit and improving the outcomes. Intralipid 10% and 20% are lipid emulsions, widely used in parenteral nutrition. Despite sever...

Inhibitors of Angiotensin-converting Enzyme or Blockers of Angiotensin-2 Receptor in COVID-19 Patients with Comorbid Cardiovascular or Pulmonary Diseases

Following the skyrocketing spread of SARS-CoV-2 into almost all the countries over five continents, diverse clinical strategies are urgently needed to defeat its pandemic, considering that an magic-bullet antiviral vaccine or treatment is presently unavailable. WHO later proclaimed the viral outbreak as a pandemic. Despite this fast speed of the pandemic, any recommended treatment must first...
